Duties:

• Provides clinical and basic administrative support while working collaboratively in an interdisciplinary coordinated patient care delivery model.

• Performs venipuncture, phlebotomy, and capillary blood glucose readings using approved methodology for sample collection to include the importance of proper patient identification. Properly identifies all patients with two identifiers to assure the accuracy of the specimen collected and documentation of the collection is appropriate.

• Performs direct patient support work which includes but is not limited to providing personal patient care, collecting medical history, obtaining vital sign readings, obtaining 12 lead EKG tracings for reliable interpretation, completing preventative health screenings via clinical reminders, performing clinical cardiac medical treatments/procedures, and documenting care provided, reporting findings of concern to the physician or other appropriate staff.

• Identifies and reports data relative to more complex, non-routine patient situations including subtle changes in patients' behaviors or conditions/variances related to cardiac, respiratory, or neurological signs, vital signs, or other parameter.

• Completes inventory to maintain basic work clinical supplies.

• Prepares the clinical area for individual procedures.

• Prepares patients for health care appointments/interactions by tele-video, face-to-face, and other nonface-to face interactions.

• Interprets requests or instructions for medical or health care personnel.

• Provides counsel on health-related subjects as provided by physician and/or health care team members, including but not limited to recommended plan of care, post-procedure care, etc.

• Independently explains procedures and positions patient for a variety of clinical procedures.

• Responds to emergency situations such as cardiopulmonary failure, hemorrhaging, and shock by assisting in the performance of resuscitation measures.

• Collects data from patient's records and prepares and presents summary information to physician and/or medical team as indicated.

• Operates and maintains medical instruments and equipment for diagnostic and treatment procedures.

• Collects data from patient's records and prepares summaries and reports in response to a variety of requests.

• Reviews and screens requests for information.

• Identifies inconsistencies or discrepancies among documentation and discusses with appropriate staff members.

• Determines nature of inquiry and independently responds to all administrative questions.

• Obtains release forms.

• Ensures encounter forms are completed.

• Verifies and updates demographics and insurance information.

• Other duties as assigned in support of facilitated patient care.

This is a part-time weekend Saturday position.

Requirements:

• Subject to background/security investigation.

• Successful completion of preemployment reference checks.

• Upon employment, required to complete online boarding (electronic training) and on site-training

• Completion of Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification with periodic refresher review as stipulated by the organization.

Job Summary

We are looking for a dedicated and compassionate Licensed Practical Nurse to provide safe, direct patient care under the supervision of a physician. You will record medical histories, monitor patient vitals, administer medications, obtain EKGs, and assist with daily treatments to ensure high-quality patient outcomes.

Responsibilities

  • Vital Signs & Monitoring : Take and record patient measurements including blood pressure, pulse, temperature, and respiration, reporting any drastic changes.
  • Medication Administration : Administer prescribed oral and injectable medications and monitor patient responses.
  • Patient Care & Treatments : Independently perform quality EKGs with accurate lead placement for reliable interpretation. Administer insulin if/as directed by established physician directed protocol. Accurately perform vitals with good clinical acumen to reliably and professionally alert cardiologist in a timely manner as indicated by patient 's condition.
  • Obtains EKG : Knowledgeable and professionally apply EKG with up-to-date knowledge of appropriate lead placement for accurate diagnostic testing and interpretation.
  • Documentation : Maintain accurate, up-to-date patient records and notes in the electronic health record system.
  • Patient Education : Provide emotional support and educate patients and families on home care and treatment requirements.

Qualifications

  • Education : Graduate of an accredited state-approved practical nursing program.
  • Licensure : Active, unrestricted state LPN license.
  • Certification : Current Basic Life Support (BLS) and CPR certification.
  • Skills : Strong clinical judgment, effective communication, and empathy for patient care. Current EKG application and independent strong ability to obtain a reliable EKG for accurate cardiology interpretation. Good communication skills for effective communication with patients, and basic computer skills for Telehealth operation.
